The lighting in the background is a low key lighting this is made effective because we all know that the fumes from cigarettes are grey and therefore this links into the lighting, the lady in the background looks lonely-eyes big and bold with a pale face-gives a impression that she is very lonely and despite the fact that she is trying to attract attention (looks like she is only wearing a vest- low/corner of shoulder).
